Most chords can be looked up using a bunch of free chord finder sites on the net. I also recommend the Jazz Piano book by Mark Levine, it shows you lots of great shapes. If that H is actually a B its just a Em7add11, just not whacking the 9th (f#) in there. Dont get too bogged down on chord names anyway, the sounds what counts
